Wolfshead Enchant questions.

I learned a while back that this is something you get through a quest, and not just plain old enchanting, so I had some questions since I have no idea where to start with it.

Where does it start? I'm not exactly looking for a full on guide, but at least knowing where to begin is fine.

Are there any reagents required? I'm a bit poor and not really willing to drop all my 100 or so gold on bumping up my DPS a bit.

Will this be permanent? I mean- I know it will, but say I wanted to put it on my current helm but I got an upgrade, would I be able to by repeating the quest or handing in more of a certain item? It'd be pretty disappointing to know that my all time bis enchant is also one of a kind.
